Chronic Medical Case Management Services for Children
As a division of our Children's Targeted Case Management Services we are specializing in Chronic Medical Case
Management Services for Children.
We are able to provide this service for children and young adults up to the age of 21. The requirements for
this service are: a diagnosis of an autoimmune disease, diabetes, respiratory disorder, a neurological disorder,
brain injury or other chronic condition specifically recognized by the office of MaineCare.
A physical condition or the presence of conditions suggestive of damage to the central nervous system or of later
atypical physical development, such as, but not limited to, cerebral palsy, meningitis, heart defects, or
Similar to our Children’s Targeted Case Managers, our Children’s Medical Case Managers are able to provide ongoing
assessments in order to connect families with appropriate resources and programs, such as:
- Educational Appointments (IEP/504 Meetings)
- Medical/Dental Appointments and Referrals
- Behavioral Health Appointments/Referrals (Medication Management, Outpatient Therapy, In-Home Support Services, Parenting and Educational Workshops)
- Sensory Integration Support Referrals
- Financial Support
- Housing Assistance
- Respite Information
- Recreational Information (as available)
Children's Medical Case Managers are also able to provide additional support and care needed specifically geared toward the client's medical needs.
Clients must also have MaineCare or Katie Becket Insurance (if assistance is needed to obtain MaineCare or Katie Becket, please contact the office).
